Check-in Information

Check-in will be in the foyer of the UW Indoor Facility (Directions). Check-in times are Friday 3-11pm and Saturday 7-11am. Please make sure you have the following:

• Player cards and roster
• Individual players need to be checked-in and marked with their team identification before they can play
• Insurance and medical release form information for non club players (player without player card)

  Players without player cards and are not currently on a club team must each fill out a release form and purchase insurance for the weekend. An additional cost of $7 per player will be needed to cover insurance cost for the tournament weekend!

  Release form and $7 fee must be turned in at check-in.

• Please complete a Guest player form for players with a current player card but who are not playing on their current roster.

Clinic check-in will run from 4:00-4:45pm. The clinic will start promptly at 5:00pm. Please have each clinic participant complete the clinic waiver form with their $10 participation fee before arriving at the clinic check-in. Waiver forms will be turned in before the clinic at registration.
